Forms specific to enrolment

course enrolment form is to be completed by students who:

  • have accepted a graduate research degree offer and wish to enrol into their new course
  • will be revising their submitted thesis

This form is to be completed by those wishing to convert between full-time/part-time and/or internal/external study.

If you are applying for external candidature, please ensure your supervisor has completed copy of this document and it is attached to your application

This form should be completed by the main supervisor in support of the student’s application for external enrolment.

At any time during your studies, you are welcome to apply for a period of leave. For details on what you may apply for, please refer to the relevant Handbook.

In advance of your milestone presentation, you should provide your report and relevant documents to the milestone panel using this form. Please speak to your academic unit to confirm the initiation process of your milestone review.

This form can be used by students appealing a decision to terminate their enrolment

It is common for your supervisory team to change during your studies. When this occurs, you must inform us by submitting this form.

**Scholarship holders only: Please refer to the RTP Procedures for information and possible implications to your scholarship. In particular, Co-Funded Scholarships.

This form is to be used by students to apply for:

  • Recognition of prior formal, informal or non-formal learning in relation to training activities offered as part of the Monash Doctoral Program (MDP)
  • Substitution of professional development training activities where a student wishes to undertake alternative professional development activity/activities outside their program

Please refer to the relevant procedure for information.

If you are travelling overseas for reasons related to your research (i.e. conducting fieldwork, presenting at a conference) you may apply for a period of study away, with or without a travel grant. Please ensure you attach a completed Travel Plan with your application.

During your studies it is possible that continuing your research may not be possible (i.e due to personal circumstances, research facilities unavailable). In these situations, it is wise to consider all options available to you, including withdrawing from your course in good standing (without academic penalty).

Following your withdrawal, if you wish to return to your research degree, you may apply for re-admission.

If you are required to relocate interstate or overseas, and you have provided a full first draft of your thesis to your main supervisor, you may apply for Writing Up Away.

If you are returning to your Monash campus after a period of research overseas or your internship has concluded, please confirm by submitting this form.

If the research you complete for paid work and your thesis overlaps, it is important to make particular declarations as to how the data will be used. This form should be submitted as early as possible once an overlap is recognised.

This form is to be used by:

  • currently-enrolled PhD students who have been confirmed by Monash Graduate Research Office as eligibility to undertaken an internship.
  • GRIP students to advise the Monash Graduate Research Office of their industry placement.

**Please submit this form once a signed internship agreement is available.

Forms to declare your intellectual property

An intellectual property and ethical research declaration must be completed by all graduate research students.

If you are a new student and are required to assign your Intellectual Property (IP) to the University, you will be unable to progress your enrolment until you have submitted the Student Assignment Deed Poll.

Forms specific to scholarship management

This form is to be completed by commencing scholarship recipients, existing students wishing to change banking details and/or address details, or students awarded a PPA.

This form is to be completed to request setting up, extending and changing existing Monash Departmental Scholarship, Monash Research Scholarship and Donor/Named Award.

This form is to be completed to request a research allowance for Monash Silver Jubilee scholarship, Max King scholarship, and Sir James McNeill scholarship awardees.

This form is to be completed to request an extension to your central scholarship/s.

Forms to submit when there are changes to your project or program

If substantial changes have been made to your research project you can notify Graduate Research by submitting this form.

Please note: If you wish to only amend the thesis title on your record, this will be done automatically when you submit your thesis for examination.

If you are the citizen of or your project involves an entity or citizen of an Australian sanctioned country, you can submit this form when there are changes to your research project.

If you wish to change your Monash Doctoral Program, you can apply using this form.

Forms related to academic progress

Typically, students’ progress is assessed via formal milestone review panels. If for any reason a student’s research is not progressing and you wish to address this outside of the milestone review process specific resources are available to guide you.
